Treatment: Most Blue Topaz have been irradiated and or Heated for color, this one has likely also been treated.
This came from old gold jewelry. It has small chip on the crown and a few tiny other ones around,(photos 5-6) but it‘s still a pretty stone, It may be good enough to set as is or it may need to be re-cut (depending upon personal preference)
